The Nurse Practitioner will provide comprehensive primary care to seniors within a collaborative, multidisciplinary team environment. Responsibilities include managing patient referrals, coordinating hospital and home health services, and maintaining efficient clinical documentation.
Candidates must hold a Master's Degree in Nursing with board certification and an active, unrestricted NP license in the state of practice. Proficiency in both English and Spanish is required, along with experience in value-based care or primary care settings.
As a Nurse Practitioner (NP) at CenterWell Senior Primary Care, you will be part of a collaborative, multidisciplinary team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.
