They will also collaborate with an interdisciplinary team to manage patient care, reduce hospitalizations, and ensure high-quality clinical outcomes.
Candidates must hold a Master’s degree in Nursing and an active, unrestricted Advanced Practice Nurse license. At least 2 years of clinical experience in geriatrics, long-term care, or post-acute settings is preferred.

Position Summary
