The coordinator identifies patient interests and barriers to daily living activities to create and model individualized engagement and care strategies. They collaborate with the interdisciplinary team and provide ongoing support to family caregivers and facility staff through site visits and follow-ups.
Candidates must hold a current Certified Nursing Assistant (CNA) certificate and possess a valid driver's license with insurance. One to two years of CNA experience and experience working within an interdisciplinary team are preferred.
Haven Hospice
The Dementia Program Activities Coordinator will work with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias. The Coordinator models activities to family caregivers and facility staff. The Coordinator will also perform bathing consultations in which they will model strategies to maximize the comfort of both the person with dementia and their caregiver. Additionally, the Coordinator will function as an IDT member for the patient, providing information to other care team members during IDT and individually, as appropriate.
EDUCATION/EXPERIENCE• High School diploma or GED equivalent, preferred.• One (1) to two (2) years CNA experience preferred.• Experience working with interdisciplinary team preferred.
LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS• Current Certified Nursing Assistant Certificate required.• Driver's license and insurance required.
KNOWLEDGE/SKILLS/ABILITIES• Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.
TRAVEL REQUIREMENTSPercentage of Travel: 25-75%
