CVRC provides case management for individuals residing in the Central Valley, covering six counties that include Fresno, Tulare, Merced, Kings, Mariposa, and Madera.

This position provides nursing assessments and evaluations, consultation on health issues to service coordinators, and reviews cases and identifies appropriate health service needs. Participates in community resource access and liaison tasks.
