The PRN Social Worker will support clients and families by navigating psychosocial needs and coordinating community resources. Responsibilities include conducting assessments, facilitating care planning, and maintaining accurate, audit-ready documentation.
Candidates must hold a BSW or MSW from an accredited program and possess current Arizona social work licensure. Previous experience in home health, hospice, or medical social work is preferred, along with strong clinical and communication skills.

As a PRN Social Worker supporting home health, you will help clients and families navigate psychosocial needs and community resources to promote safe, appropriate care planning—while working fully remote. Key responsibilities include completing psychosocial assessments, identifying barriers to care, coordinating referrals to community services, supporting advance care planning discussions as appropriate, and documenting thoroughly within required timeframes.
Required qualifications/skills:
- BSW or MSW from an accredited program
- Current Arizona social work licensure (as applicable)
- Home health/hospice or medical social work experience preferred
- Strong clinical judgment, communication, and documentation skills
- Ability to manage PRN caseload needs with reliability in a remote setting
