The RNAC is responsible for coordinating the resident assessment and care plan process in compliance with federal, state, and local regulations. This role also manages Medicare and managed care assessments while ensuring survey readiness and accurate documentation.
Candidates must hold a current RN license and possess strong knowledge of MDS, RAI, and care planning processes. An associate or bachelor degree in nursing is preferred, along with experience in resident assessment and care planning.

We are currently seeking an experienced Registered Nurse Assessment Coordinator- (RNAC) to join our collaborative team to support regulatory compliance, resident centered care and survey readiness.
You will have the opportunity to work with a great team of dedicated individuals. We offer a team based approach to patient care and operate in a supportive environment that fosters the highest level of teamwork, integrity, respect and empowerment.
This role is responsible to conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility. Strong knowledge of MDS, the RAI and care planning process, including the implementation of CAAs.
Coordinates scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews in accordance with the most recent regulatory guidelines.
Inform residents, responsible parties and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state and facility guidelines. Provides updates as necessary. Works with Community Health Choice (CHC) case managers for Managed Medicaid providers Participate in committees as directed.
In-depth knowledge of Pennsylvania case mix, Medicare PDPM payment system and requirements for managed care coordination. Knowledge of Quality Measures.
