The Case Manager provides assessments, develops individualized case plans, and monitors progress for participants in facilities and programs. They collaborate with community providers and government agencies to advocate for participants and facilitate transitions into sustainable housing.
Requires a Bachelor's degree or two years of related work experience, along with a valid driver's license and a commitment to the organization's Christian faith. Preferred qualifications include CAS certification and the ability to obtain CPR certification.

The Case Manager (CM) should be someone who takes initiative, can multi-task, and is able to manage a large caseload. The CM provides assessments, assignments, and accountability to guests and participants in our facilities and programs.
