Seeking a dedicated and compassionate Family Nurse Practitioner to provide and manage a high standard of healthcare for patients in Aurora, CO. The successful candidate will be responsible for delivering comprehensive care to patients of all ages.
Key Responsibilities:
Provide comprehensive primary and family healthcare services to patients across the lifespan.
Perform health assessments, physical examinations, diagnosis, and treatment planning.
Diagnose and manage acute and chronic medical conditions.
Order and interpret appropriate laboratory tests, imaging, and diagnostic studies.
Prescribe medications and treatments within state and federal guidelines.
Provide preventive care, health education, wellness counseling, and screening.
Develop individualized care plans in partnership with patients and their families.
Educate patients about diagnoses, medications, lifestyle changes, and preventive health.
Monitor patient progress and adjust treatment plans as clinically appropriate.
Maintain accurate, timely, and thorough electronic medical records.
Coordinate care and make appropriate referrals to specialists and other healthcare professionals.
Collaborate with the broader clinical team to ensure safe, high-quality, coordinated care.
Participate in quality improvement initiatives and help continuously improve the patient experience.
