The Care Coordinator conducts in-person face-to-face assessments for clients in the South Central Richmond area. They are responsible for coordinating behavioral health services and assisting vulnerable populations with care transitions and service navigation.
Candidates must have a Bachelor's degree in Health & Human Services or a related field and at least one year of experience in social work, care coordination, or behavioral health. Experience with ID/DD and substance use disorders is preferred.
Hours: Full Time (40 hrs/wk)
Shift: Day (8am-5pm)
