The Nurse Liaison provides health assessments, education, and therapeutic interventions while serving as a bridge between physicians and interdisciplinary teams. They are responsible for developing individualized treatment and discharge plans and managing client caseloads.
Candidates must have a Bachelor's degree in Nursing or Behavioral Science, a MN Registered Nurse (RN) license, and BLS certification. At least 3 years of experience in a mental health or related setting is required.
Provides assessment, education and therapeutic interventions related to health/wellness issues for clients and their families. Serves as a liaison between the physician and the interdisciplinary team. Effectively communicates verbally and in writing with the interdisciplinary team and external support systems. Participates in the development and implementation of the treatment and discharge plan. Functions as an interdisciplinary team member. Responsible, as assigned, for the management of caseload.
