Overview
POSITION SUMMARY:
Under the supervision of the Medical Director, provides primary care to participants. Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care for program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities. Provides in home visits during the day as needs arise.
The PACE Program is defined as the Program for All-Inclusive Care for the Elderly. PACE is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.
Monday-Friday role with flexibility and shaping
Will have mixture of home visits and time at PACE center in Peoria

Qualifications
REQUIRED QUALIFICATIONS
Master’s degree or higher in nursing or physician assistant studies from an accredited program
Current Illinois license as a nurse practitioner or physician assistant
Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
Minimum of two years of experience as an APP in geriatrics or a related field is preferred
Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
Proficiency in electronic medical record (EMR) systems

Demonstrates the knowledge and skills necessary to provide patient care aligned to the preferences, goals, and needs of the patients served
