Responsibilities
The RN Care Manager is responsible for patient assessment, care planning, and ongoing evaluation to ensure medically and financially appropriate care. This includes coordinating with an interdisciplinary team and managing participants recently discharged from hospitals or skilled nursing facilities.
Requirements
Candidates must possess a Washington Registered Nurse License, BLS certification from the American Heart Association, and at least one year of experience in long-term care or geriatric nursing. A Bachelor's Degree in Nursing and experience in medical-surgical or rehabilitation medicine are preferred.

The Registered Nurse Care Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.

Required Qualifications:
Education sufficient for clinical licensure.
Upon hire: Washington Registered Nurse License
Upon hire: National Provider BLS - American Heart Association
1 year Long term care or geriatric nursing.
