The Social Worker will serve as a vital member of the patient care team, supporting patients through the physical, mental, and financial challenges of End Stage Renal Disease. They will build long-term relationships with patients and their families while helping them navigate their rights and responsibilities in an outpatient setting.
Candidates must hold a Master's degree in Social Work (MSW) with a specialization in clinical practice and meet all state regulations for dialysis practice. Proficiency in Microsoft Office applications and knowledge of government and private insurance programs are also required.
This part time (24 hours) role will be split between two facilities.

In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements:
Meets all state required regulations to practice in the dialysis setting.
Master's degree in Social Work (MSW) required with a specialization in clinical practice
Demonstrated knowledge of government and private insurance programs
Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
