MassAbility is seeking a dynamic individual to fill the role of Waiver Program Coordinator. The selected candidate is responsible for needs assessment, service plan development, service authorization activities, and implementation and monitoring of individualized service plans (ISP) for Acquired Brain Injury Non-Residential waiver (ABI-N) and Money Follows the Person - Community Living Waiver (MFP-CL) enrollees/participants statewide, as well as for MFP Demonstration participants.
Waiver Program Coordinator engage in outreach as assigned to ensure that potential applicants are aware of the waivers and supported in the application process. For each assigned enrollee/participant, the Waiver Program Coordinator coordinates an interdisciplinary team that includes the individual, his/her guardian, if any, relevant waiver service participant waiver service providers, other persons as chosen by the enrollee/participant and other appropriate professionals to develop an Individual Service Plan (ISP) utilizing a person-centered planning process. The planning process will determine what ABI-N and MFP-CL waiver services and supports are needed to support the waiver participant to live safely in the community, which are delineated and authorized in the ISP portion of the Person-Centered Plan (PCP).

Please note: Applicants will need access to reliable transportation as statewide travel is necessary to the performance of this position regardless of location. Travel is expected in order to cover caseload assignments. This position is based in Worcester.
Duties and Responsibilities (these duties are a general summary and not all-inclusive):
Engage assigned enrollees/participants, their guardians and families as appropriate, in a person-centered planning process with careful consideration and accommodations for communication or accessibility needs.
Meet in-person with the enrollee/participant to perform or arrange for any specialized assessments identified as needed pre-discharge, in conjunction with the enrollee/participant, his/her legal guardian, if any, and other appropriate professionals.
Develop a person-centered plan (PCP) for assigned enrollees/participants that include services and supports and other community resources (ISP) that will be used to address their needs.
Partner with the individual and/or their family and MassAbility Adaptive Services Coordinator to ensure that approved home accessibility modification projects meet the needs of the individual and are satisfactorily completed in a timely fashion.
Coordinate an interdisciplinary team process for developing each Individual’s Person-Centered Plan (inclusive of all plan sections) that includes the participant, his/her guardian if any, relevant waiver service providers, other persons as chosen by the Individual and other appropriate professionals.
Update any assessments, as necessary, including any specialized assessments, and arrange for the provision of any such special assessments.
Safeguard the ABI-N /MFP-CL waiver services and supports identified in the plan, as necessary to support the waiver participant to live safely in the community, are delivered and are satisfactory.
Arrange for referrals and provision of MassHealth Long-Term Supports and Services (LTSS) and other needed services by selected providers; monitor and ensure service delivery, and ensure linkage with community resources, including their medical providers.
Conduct outreach in nursing facilities and rehabilitation hospitals as assigned to recruit waiver participants, and to educate and inform the facility staff about the Home and Community Based Waivers, particularly underutilized waivers.
Work collaboratively with the agency/clinicians to further assess and determine strategies for successfully addressing the individualized, special needs of the individual.
Required Qualifications:
Ability to travel statewide is required.
Excellent communication skills in oral and written form, solid follow-through, flexibility.
Ability to clarify and to resolve problems and conflicts, and the ability to self-organize and manage priorities.
Ability to work comfortably with remote technology (computer, VPN, remote calling) is required.
Familiarity with the needs of individuals with disabilities living in the community is a must.
Preferred Qualifications:
Knowledge and applicable experience working with individuals with disabilities and their families, preferably those with Acquired Brain Injury.
Knowledge of Medicaid, state agencies and the provider service system and community-based resources available to serve persons with disabilities or elders that support their transition to independent living.
A minimum of two years of experience with transitioning individuals from a nursing facility or long-stay hospital to community-based settings.

MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) three years of full time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, or (B) any equivalent combination of the required experience and the substitutions below.
Substitutions:
I. A Bachelor's degree with a major in business administration, business management or public administration may be substituted for a maximum of two years of the-required experience.
II. A Graduate degree with a major in business administration, business management or public administration may be substituted for the required experience.
III. A Bachelor's or higher degree with a major other than in business administration, business management or public administration may be substituted for a maximum of one year of the required experience.
Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.
Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
