The Inpatient Care Navigator performs comprehensive psycho-social assessments and coordinates discharge planning to ensure a smooth transition for patients. They collaborate with multidisciplinary teams to identify and resolve barriers to patient care and discharge.
Candidates must hold a valid RN license or Social Work degree (BSW/MSW) with 1-3 years of clinical experience for nurses or 3 years for social workers. Proficiency in EMR systems and active BLS certification are required for this role.

Primary weekends: Every other weekend (Saturday and Sunday), or every Saturday, or every Sunday, as scheduled
Orientation is expected to be approximately 3–4 weeks and can be flexible (e.g., one week on, one week off).
Hours: 8:00 AM – 4:30 PM
Major holidays are required based on departmental needs.
RN Per Diem Rate:$48
SW Per Diem Rate: $45
If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.

Tobacco use is a well-recognized preventable cause of death in the United States and an important public health issue.
