The Clinical Case Manager supervises home care personnel and ensures compliance with state and federal regulations. They are responsible for developing patient treatment plans, coordinating care, and managing clinical documentation for reimbursement.
Candidates must hold a current state license as a Registered Nurse, with a BSN preferred and a current CPR card. A minimum of two years of nursing experience in home health or acute settings is preferred.
