At Advocate Support Services, Inc., our Case Workers provide community-based Psychiatric Rehabilitation Program (PRP) services to adults living with serious mental illness.

What You’ll Do
As a Case Worker, you will manage an assigned caseload of adults receiving PRP services and provide individualized rehabilitation services in homes and community settings.
Your responsibilities will include:
Meet with clients regularly in their homes and communities to provide skill-building and rehabilitation services
Help clients develop practical skills related to:
Activities of daily living
Coping skills and emotional regulation
Medication and health-management routines
Budgeting and financial organization
Transportation and community navigation
Social skills and community integration
Appointment follow-through
Accessing community resources
Independent living and personal goals
Develop meaningful, professional relationships that promote engagement and progress
Maintain consistent contact with assigned clients and proactively address missed or difficult-to-schedule visits
Plan and organize your weekly schedule to meet client needs and program expectations
Coordinate with therapists, psychiatrists, healthcare providers, family members, and community partners when appropriate
Work collaboratively with Rehabilitation Specialists and program leadership
Support implementation of each client’s Individual Rehabilitation Plan (IRP)
Complete accurate, individualized documentation in the electronic health record
Complete documentation promptly and in accordance with program and regulatory requirements
Participate in supervision, team meetings, training, and quality-improvement activities
What Success Looks Like
Successful Case Workers at Advocate:
Build genuine relationships with clients
Consistently complete scheduled client encounters
Manage approximately 20 active clients in a full-time caseload
Organize their schedules effectively and maintain consistent client engagement throughout the month
Complete documentation accurately and on time
Communicate proactively when clients disengage or barriers arise
Work independently without becoming disconnected from the team
Balance compassion with accountability
Understand that reliability and follow-through are essential parts of helping clients succeed
What We’re Looking For
You may be a strong fit if you:
Have experience working with adults with serious mental illness, behavioral health needs, developmental disabilities, substance use disorders, homelessness, case-management needs, or other vulnerable populations
Enjoy working directly with people in their homes and communities
Are comfortable spending much of your workday traveling between client locations
Can build rapport with people from different backgrounds and life circumstances
Are dependable, organized, and able to manage your own schedule
Communicate professionally and respectfully
Can maintain appropriate professional boundaries
Are comfortable using technology and electronic health records
Can write clear, accurate behavioral health documentation
Respond well to coaching, supervision, and performance expectations
Required
Valid driver’s license
Reliable personal transportation
Ability to travel throughout assigned Maryland service areas
Ability to perform a primarily mobile, community-based position
Ability to meet applicable employment, background-screening, and program requirements
Preferred
Previous Psychiatric Rehabilitation Program (PRP) experience
Experience working with adults with serious mental illness
Behavioral health, human services, social services, psychology, rehabilitation, or case-management experience
CPRP certification or interest in professional rehabilitation certification
Bilingual or multilingual skills
Schedule & Work Environment
This is not a traditional office-based case-management position.
Case Workers spend the majority of their service time working directly with clients in community settings.
The position includes:
Full-time employment
Flexible field scheduling based on client needs
Community-based client visits
Limited telehealth services when appropriate and consistent with program requirements
Weekly supervision and ongoing support
Electronic documentation
Regular team collaboration
Because clients may have different schedules and availability, successful Case Workers must be able to organize their workday around client engagement rather than expecting a traditional desk-based 9-to-5 schedule.
Compensation
$22.00–$24.00 per hour, based on qualifications and relevant experience.
Annualized equivalent: approximately $45,760–$49,920 based on a standard 40-hour workweek.
This is a full-time, non-exempt position and is eligible for overtime in accordance with applicable law and Advocate policy.
