The Social Worker will support dialysis patients at skilled nursing facilities by helping them manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. They will act as a vital member of the patient's core care team, advocating for patient rights and building long-term relationships.
Candidates must hold a Master's Degree in Social Work (MSW) and possess a valid clinical license such as LCSW or LICSW. Proficiency in social work duties, intermediate computer skills, and knowledge of government and private insurance programs are required.
The Social Worker must have obtained an MSW and meet the clinical license requirements for the state of AZ (ex. LCSW, LICSW)
The Social Worker will support DaVita Skilled Nursing Facility Dialysis (SNF) patients. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside, instead of being transported to an outpatient dialysis center.

What you can expect:
The MSW currently is likely to have a starting caseload of around 110, but this has the potential to grow. The MSW can work a hybrid schedule, but must be located in the Phoenix metro areaThe number of days on-site may vary. It is likely to be about two days per week from home and three days on-site meeting with patients at our Arizona Skilled Nursing Facility Dialysis locations. The MSW is likely to support patients at the following sites:
