Posting Date
08/26/2026337 East Coronado RoadSuite 101, Phoenix, Arizona, 85004, United States of America
The Social Worker must have obtained an MSW and meet the clinical license requirements for the state of AZ (ex. LCSW, LICSW)
The Social Worker will support DaVita Skilled Nursing Facility Dialysis (SNF) patients. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside, instead of being transported to an outpatient dialysis center.
As a Social Worker at DaVita, you’ll be a part of a Team that values work-life balance and where your personal and professional growth is a top priority. DaVita has an open position for a Social Worker who will be a vital member of each patient’s core care team. Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
If you love patient-centered health care and knowing patients on a personal level—now is your time to explore your next journey—at DaVita.
What you can expect:
Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
Be a part of a Team that appreciates, supports and relies on each other in a positive environment.
Performance-based rewards based on stellar individual and team contributions.
The MSW currently is likely to have a starting caseload of around 110, but this has the potential to grow. The MSW can work a hybrid schedule, but must be located in the Phoenix metro areaThe number of days on-site may vary. It is likely to be about two days per week from home and three days on-site meeting with patients at our Arizona Skilled Nursing Facility Dialysis locations. The MSW is likely to support patients at the following sites:
