The Behavioral Health Case Manager provides proactive interventions, assessments, and individualized care plans for patients at the Primary Care Clinic. They coordinate with multidisciplinary teams to link patients with specialty care and ensure retention in behavioral health services.
Requires an advanced degree in social work or mental health counseling and at least two years of related experience. Candidates must obtain LMHC or LMSW licensure and UAS-NY certification shortly after hire.

Position summary: The Behavioral Health Case Manager is responsible for ensuring high quality behavioral health interventions and services to patients of Apicha’s Primary Care Clinic to ensure access to and retention in care.
