About the Role: The Behavioral Health Care Coordinator plays a pivotal role in managing and enhancing the delivery of behavioral health services within DPHS. This position is responsible for coordinating entry into behavioral health services, ensuring timely access, facilitating communication among multidisciplinary teams, and ensuring that patients receive comprehensive behavioral health support. Ultimately, the role contributes to improved mental health and overall well-being of individuals served.
Minimum Qualifications:
Master’s degree in social work, counseling, psychology, or a related field.
Must be license eligible.
Strong knowledge of behavioral health conditions, treatment modalities, and community resources.
Familiarity with healthcare documentation standards and electronic health record (EHR) systems.
Excellent communication and interpersonal skills to effectively collaborate with diverse stakeholders.
Preferred Qualifications:
LCSW or LPC
Experience working within integrated care or multidisciplinary healthcare teams.
Bilingual proficiency to assist a diverse patient population (English/Spanish)
Responsibilities:
Provide open-access triage, education, and immediate, brief evidence-based interventions to patients and families across clinic settings (including medical, dental, and waiting areas), while managing a small, dedicated caseload for short-term supportive counseling.
Develop and implement individualized behavioral health care plans in collaboration with healthcare providers and patients.
Coordinate communication and information sharing among multidisciplinary teams including psychiatrists, therapists, social workers, and primary care providers.
Monitor patient progress and adherence to treatment plans, adjusting coordination efforts as needed to optimize outcomes.
Facilitate referrals to community resources, support services, and specialty care to address social determinants of health impacting behavioral health.
Maintain accurate and confidential patient records, ensuring compliance with healthcare regulations and organizational policies.
Identify and address barriers to care such as transportation, insurance issues, or social challenges.
Participate in quality improvement initiatives aimed at enhancing behavioral health service delivery and patient satisfaction.
