Child & Adolescent Intensive Care Coordination Case Manager
Annual Salary - $53,569
Work Schedule: Monday - Friday 8:30 am - 5:00 pm (Some on-call responsibilities)
Case Management is a vital part of the H-NNCSB service delivery system, serving as a unifying link between families, children, and the supports they need. The Intensive Care Coordination (ICC) Case Manager offers a higher level of support, including clinical assessments and level-of-care determinations, for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD). This role exists to ensure services are timely, appropriate, and aligned across schools, providers, and community supports.
Key responsibilities include completing clinical and service assessments, developing individualized service plans, coordinating referrals and authorizations, tracking progress toward goals, maintaining accurate documentation, and providing advocacy and crisis response as needed in partnership with the multidisciplinary team.
Required qualifications/skills: - Bachelor’s degree in a Human Services field such as Social Work, Psychology, or Counseling - Two (2) years of experience providing mental health case management or care coordination services for children with knowledge of DSM-5 diagnosis and care management principles
