The Case Manager provides direct support and case management to adults with serious mental illness to assist them in living independently. Responsibilities include implementing recovery plans, documenting interactions, advocating for members, and assisting with daily living needs.
Candidates must have a high school diploma or G.E.D. and be at least 18 years old. Required certifications include CPR, First Aid, and Crisis Prevention Institute (CPI) training, along with a valid driver's license.

POSITION SUMMARY
Under direct supervision of the Team Leader, serves as the provides a support and case manager for members served with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Full-Time | AM | Shifts: 7:00 AM - 3:30 PM | Days: Friday - Monday
Expected starting wage range is $23.00. This position is covered by a collective bargaining agreement and is subject to union membership requirements upon hire. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to stand, twist, squat, kneel, push, pull and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend and do simple and power grasping. The position requires to drive and frequent Hazardous Exposure (Blood Borne Pathogens, hospital waste, Chemicals & Infections).
