A bachelor's degree in social work, psychology, or a related field is required along with at least one year of care management experience. Preference is given to candidates with experience supporting individuals with mental health needs or intellectual and developmental disabilities.

OVERVIEW OF PRIMARY RESPONSIBILITES:
The Care Manager within the Critical Time Intervention (CTI) program plays a vital role in supporting individuals transitioning from institutional settings, such as hospitals, into the community. This position ensures continuity of care by providing intensive case management services and working collaboratively with internal CTI teams, hospital discharge teams, and community providers. The role emphasizes proactive coordination of mental health, housing, and developmental disability services while advocating for participant independence and long-term stability.
