The Housing Case Manager provides comprehensive case management and care coordination for residents living with HIV and co-occurring conditions. Responsibilities include supportive counseling, crisis intervention, and conducting monthly home visits to ensure housing stability.
A minimum of three years of relevant social services experience working with homelessness, mental health, or substance use is required. A bachelor's degree in social work, psychology, or counseling is preferred, and a master's degree is a plus.

Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to a caseload of residents who are living with HIV and/or and co-occurring conditions. The Housing Case Manager works closely with their clients in the clients’ homes, in the office, and in the community helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.
