Benefits: We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
Overview:
The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to a caseload of residents who are living with HIV and/or and co-occurring conditions. The Housing Case Manager works closely with their clients in the clients’ homes, in the office, and in the community helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.
Responsibilities:
HCM provides case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, field work, and assessment/observation of resident in their living space as needed but no less than once per month.
Documentation of all services and interventions provided as per funder, regulatory, industry and internal standards.
Requirements:
Bachelor's degree in social work, psychology, counseling, or a related field preferred.
Master’s Degree a plus.
A minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use is required
