The RN Care Manager develops and implements comprehensive Member Care Plans by conducting health assessments and coordinating with primary care providers. They provide essential education on disease processes and lifestyle modifications while maintaining required documentation for DHS contracts.
Candidates must be a Licensed Registered Nurse in Wisconsin with at least an associate degree in nursing. A valid driver's license, personal vehicle insurance, and a preference for one year of care management experience are also required.
Main Responsibilities:
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.
Mileage reimbursement is provided for work-related travel. Eligible mileage includes:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
