The Community Health Worker will conduct outreach and engagement activities while providing comprehensive case management and trauma recovery support to youth. They will also maintain accurate documentation and collaborate with hospital staff to facilitate warm handoffs and referrals.
Candidates must have a high school diploma or GED and at least two years of experience providing peer services to violently injured individuals. A Certified Peer Specialist (CPS) certification in Pennsylvania and fluent proficiency in both Spanish and English are required.
We are seeking a compassionate and experienced individual to fill the role of Community Health Worker Practitioner (CHWP) at the Community Outreach Team (COT) program, affiliated with the Center for Nonviolence and Social Justice at Drexel University's Dornsife School of Public Health. The successful candidate will split their time between providing support at partnering community organizations, and remote work, ensuring the delivery of comprehensive community health services. This job requires a significant amount of community-based work (travel), and a willingness to engage with a diverse range of neighborhoods in Philadelphia.
This position is grant-funded; employment is contingent upon the continued availability of those funds.
Additional Information
This position is classified as Non-Exempt, grade G. Compensation for this grade ranges from $17.17 to $25.76 per hour. Please note that the offered rate for this position will vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
