Job Summary
A Behavioral Health Care Manager coordinates mental health treatment, tracks patient progress, and links individuals to community support. They work with doctors, therapists, and patients to build custom care plans. Key duties include monitoring symptoms, supporting medication plans, and providing short-term counseling.
Description of responsibilities:
Maintain a caseload and deliver services.
Conducts screenings and comprehensive assessments for co-occurring behavioral health issues according to best practices and agency policy and procedures.
Implements and monitors treatment plans that are developed with a Person-centered/Family-centered philosophy.
Responsible for the identification of treatment referrals/resources within the community appropriate to meeting the needs of the individuals served.
Knowledgeable of motivational interviewing, engagement strategies, co-occurring disorders, and stages of change and applies principles.
Applies evidence-based practices when applicable, including the use of motivational interviewing and other strategies to encourage full participation from those receiving services.
Completes documentation and reports according to agency and funding source timeframes.
Ability to deliver services in the community.
Ensures assigned paperwork is completed and maintained in accordance with funding source standards and CARE of SEM policy and procedures.
Able to apply outcome tools to measure services; for example – self-sufficiency matrix.
Promote the program to obtain referrals and CARE of SEM’s services as needed.
Establish and maintain liaison relationships with appropriate agencies regarding intervention and clinical services.
Knowledgeable of national and local behavioral health, substance use, and prevention statistics and demonstrates the ability to apply current trends to service delivery.
Required experience:
One year of experience in treating or working with a person who has mental illness
Meets qualifications or is eligible to become a Qualified Mental Health Professional (QMHP) as defined in the Medicaid Provider Manual outlined in the Medicaid Provider Qualifications Crosswalk.
Must possess knowledge and understanding of Recovery Oriented System of Care and Trauma Informed Services.
Required skills / education:
Qualifies as a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.
