Coordinates care for mental health, behavioral, and social needs while facilitating communication between patients, families, and healthcare professionals. Responsible for implementing care plans, conducting psychological assessments, and managing patient transitions and discharges.
Requires a Master's Degree in Social Work and prefers a minimum of 2 years of related work experience. Must possess skills in psychotherapy, intake assessments, and behavioral therapy management.
