The Nurse Practitioner manages a defined patient panel, performing physical exams, ordering diagnostic tests, and formulating longitudinal care plans for patients with chronic conditions. They collaborate with interdisciplinary teams to ensure care alignment and provide patient education across home, telehealth, and clinical settings.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in primary care, cardiology, or nephrology. A current state license, national board certification, and a valid driver's license are required.

Annual Salary Range: $112,000.00-$140,000.00. This position is also eligible for a target annual bonus of 10%
