The Physical Therapist will plan and administer personalized rehabilitation programs for patients in their homes to improve strength and mobility. They are also responsible for documenting patient progress and coordinating care with the clinical team and physicians.
Candidates must hold a current and unrestricted Physical Therapy license and CPR certification. Experience collaborating with healthcare professionals is required, with one year of experience preferred.
