Provides direct social work services to pediatric and PICU patients and families, including assessments, treatment planning, and crisis intervention. Coordinates care and discharge planning while collaborating with interdisciplinary healthcare teams and community resources.
Requires a Master's degree in social work or a related discipline and an unrestricted license from the Minnesota Board of Social Work (LGSW, LISW, or LICSW). Candidates must possess strong counseling skills and proficiency in electronic health record software.
SUMMARY
We are currently seeking a Social Worker II to join our Case Management team in the Peds/PICU units. This fulltime role will work days (Monday to Friday).
Purpose of this Position: Provides direct service to clients and their families. This includes social work assessment, interventions and treatment plans, supportive counseling and crisis intervention, care management, discharge/transition planning, advocacy and support. Seeking resources and services to address unmet needs. The social worker also provides consultation services and in-service education programs related to the psychosocial aspects of health care to other professional staff members on the healthcare team.
