Our philosophy is that every personshould beafforded dignityandrespect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach,financial assistance, community connection and referral services.
About the role
As a Case Manager – Homeless Outreach Program, you will have the opportunity to provide outreach and holistic case management support to people experiencing homelessness, rough sleeping, or living in insecure accommodation. You will work with individuals who may be disconnected from mainstream services, you will deliver advocacy, housing assistance, system navigation, and connections to health and community supports. This rewarding role offers the opportunity to empower clients to improve their wellbeing, sustain housing, and build meaningful connections within their community.
Reporting to the Team Leader, this is a permanent part-time position, 22.8 hours per week, based in Nunawading, VIC.
Conditions Compensation is in accordance with SCHADS Award Level 5.

Deliver outreach and case management support to people experiencing or at risk of homelessness
Develop tailored support plans and connect clients with housing, health, and community services
Advocate for clients and help them navigate complex service systems to achieve their goals
Build positive, trusting relationships that promote independence, wellbeing, and social inclusion
Maintain accurate client records and contribute to high-quality, person-centred service delivery
What you will bring
Relevant tertiary qualifications in Social Work, Human Services, Age Care, Disability, Community Services or related field. Diploma Level minimum, or relevant experience
A current VIC Driver's Licence
A current Employee Working with Children Check
