Physical Therapist (PT)
Shifts Available: PRN & FT
Service Areas with Openings: Northeast, Northwest, Southwest
Responsibilities:
Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
Implement physical therapy care plans.
Coordinate care with the home healthcare team.
Educate patients and family members about health conditions.
Prepare timely, complete, and accurate clinical notes.
Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
Qualifications:
Physical Therapist in the state of Texas.
Home health experience highly preferred.
Knowledge of home health regulations and general healthcare compliance.
Experience with HCHB EMR. (preferred but not required)
Able to handle physical workload.
Good driving record.
Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
