The social worker is responsible for conducting psychosocial assessments and implementing care plans to ensure continuity of care and smooth transitions for neurology patients. They serve as a liaison between the healthcare team and families, addressing emotional, social, and financial aspects of care.
Candidates must hold a Master of Social Work (MSW) and be licensed by the State of Ohio Social Work Board. A minimum of 1 to 2 years of related experience is required, with 3 to 5 years preferred.
