Ennoble Care is looking for a full-time, experienced Care Coordinator, Embedded, who will work out of our client's senior living facility in Cincinnati, Ohio, who aligns with our motto, "To Care Is An Honor". This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Care Coordinator, you should be compassionate, experienced, and highly organized. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the caregivers and families.
Pay Rate: $18.00 - $19.00/hr

Ennoble Care provides home-based primary care, palliative care, and hospice for seniors who are frail, homebound, or close to it. We meet patients wherever they live, including private homes, senior communities, and long-term facilities. Our goal is to bring care to the places where it is needed most. Our clinicians visit patients many times a year, supported by daily care coordination, close observation of changes in a patient’s condition, and remote monitoring. That level of presence keeps small problems from becoming crises. As our Quality and Outcomes Report shows, hospital admissions fall 25 percent in the first year and 63 percent by the second year. Medicare hospital spend drops to roughly half that of comparable homebound populations. We serve approximately 50,000 patients across 15 states and the District of Columbia. Many live with multiple chronic conditions, cognitive impairment, and social isolation. Our work requires proximity, consistency, and a team that knows its patients well. Our mission is to provide individualized care that supports physical, emotional, and spiritual well-being. We care for patients across the full continuum, including chronic condition management, behavioral health, palliative care, hospice, and end-of-life support. Quality, cost, and access are not tradeoffs. They are the foundation of value-based care that works for the seniors who need it most. To care is an honor.
