Family Solutions is seeking a compassionate and dedicated Qualified Mental Health Professional (QMHP) to join our growing team. This position is ideal for a mental health clinician, therapist, social worker, or behavioral health professional who is passionate about providing person-centered, trauma-informed care and supporting clients in community-based settings.

What You’ll Do
Provide Comprehensive Care: Conduct person-centered, culturally responsive, trauma-informed clinical evaluations, including assessment of mental status, risk factors, strengths, needs, abilities, and client preferences.
Support Clinical Assessment: Gather relevant clinical and historical information, assess client functioning and safety, determine appropriate service needs, and formulate diagnostic impressions using the Diagnostic and Statistical Manual of Mental Disorders (DSM).
Develop Individualized Treatment Plans: Partner with clients and, when appropriate, their families or significant supports to develop individualized treatment plans focused on meaningful goals, client preferences, and medically necessary services.
Provide Therapy & Mental Health Services: Deliver outpatient therapy, mental health counseling, crisis intervention, case management, Community Support Services, treatment planning, and other behavioral health services consistent with your qualifications, licensure, and applicable standards.
Use Evidence-Based Practices: Utilize evidence-based, trauma-informed, and person-centered therapeutic interventions that are appropriate to the client and permitted by applicable regulations and funding requirements.
Respond with Care: Address client clinical and behavioral health needs in a timely and responsive manner while maintaining a strong focus on safety, engagement, and positive outcomes.
Collaborate for Better Outcomes: Work closely with clients, families, treatment team members, external care providers, schools, community agencies, and other service providers to coordinate care and support progress toward treatment goals.
Document with Purpose: Maintain accurate, timely, and compliant clinical documentation, including assessments, treatment plans, progress notes, billing documentation, and other required records.
Practice with Integrity: Maintain confidentiality, follow ethical standards, practice within the scope of your qualifications and Illinois licensure when applicable, and comply with Family Solutions policies, accreditation requirements, and applicable regulations.
Grow with Us: Participate in professional development, team meetings, quality and performance improvement initiatives, and other activities that support continuous learning and excellent client care.
Deliver Consistent Services: Meet established productivity and quality standards while providing consistent, client-centered services. The expected productivity standard for this position is an average of 28 billable hours per week.

What We’re Looking For
QMHP I — Master’s Level
Master’s Degree in Counseling, Social Work, or a Related Behavioral Health Field

Licensure: Current Illinois professional licensure, such as LPC, LSW, LMFT, or a higher-level applicable license, required.
Experience: Experience in clinical care required; experience in an outpatient or community-based behavioral health setting is preferred.
Clinical Skills: Ability to provide clinical assessment, treatment planning, individual therapy, mental health counseling, crisis intervention, case management, Community Support Services, and other clinical services within the scope of professional licensure.
Clinical Approach: Familiarity with evidence-based, trauma-informed, and person-centered behavioral health practices preferred.
Requirements for All QMHP Positions
Certifications: Ability to complete First Aid, CPR, verbal/physical de-escalation, and required professional development training.
Ability to pass required background and compliance screenings
Ability to pass employment drug screening
Ability to meet Family Solutions documentation, productivity, and quality standards
