Responsible for the assessment, planning, and evaluation of behavioral health care for adult and geriatric patients in an outpatient home care setting. Duties include case coordination, insurance precertification, and providing medication monitoring and education.
Must be a graduate of an accredited nursing program with a current Registered Nurse license and BLS certification. A valid driver's license and fluency in English are required for the role.
Responsible for the assessment, planning, implementation and evaluation of patient care in the outpatient setting for behavioral health home care. Responsible for case coordination of patients and insurance precertification/utilization review. Patients served include adult and geriatric. St. Luke’s C.A.R.E. delivery model (Continuity, Accountability, Relationship-based, Evidence-based) reflects a commitment to compassionate, competent care delivered by the interdisciplinary team. Care is provided through a therapeutic relationship with the patient and family. A spirit of inquiry is encouraged and supported. Small tests of change and more formal research efforts are used when the evidence is not clear. Evidence-driven protocols are utilized to assure reliable and consistent care.
