The Case Manager coordinates patient care from admission to discharge, overseeing interdisciplinary plan-of-care decisions and resource management. They facilitate team conferences, monitor patient experience, and ensure compliance with healthcare regulations and payor requirements.
Candidates must be licensed healthcare professionals (e.g., RN, SW, OT, PT, ST) with at least an associate degree for nursing or a bachelor's degree for other disciplines. CCM or ACM certification is required or must be obtained within two years of hire.
