The Licensed Case Manager provides assessment, treatment planning, and crisis stabilization for individuals with mental health and substance use disorders. They coordinate care with interdisciplinary teams and community partners to ensure seamless discharge and ongoing recovery support.
Requires a Master's or Doctoral degree in a behavioral health field and current Virginia licensure as an LMHP (e.g., LPC, LCSW, LMFT). Candidates must pass state background checks and be capable of performing Safe Clinch restraint techniques.

What You'll Do:
The Licensed Mental Health Professional (Licensed Case Manager II) supports clients with mental health and substance use recovery through psychoeducation, assessment, and treatment, resource connection and compassion through facilitating assessments, treatment planning, discharge planning, obtaining collateral and assisting in the legal process and initiating personalized care for individuals who are in crisis and stabilizing from crisis. Works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for Individuals, their family members, clinics, local providers, and/or other referral sources. This position will carry a caseload and act as ambassador to community partners.
