Candidates must hold a degree in Physical Therapy and possess a current, unencumbered license to practice in the state. A minimum of two years of relevant experience is required, along with valid CPR certification and reliable transportation.

POSITION SUMMARY
The Physical Therapist (PT) is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers. The PT performs a variety of tasks and is encouraged to use a certain degree of creativity and latitude.
