The Discharge Planner coordinates activities to develop and implement patient care plans post-hospitalization. They ensure continuity of care by meeting the specific needs of patients within a behavioral health setting.
Candidates must have at least one year of experience in the behavioral field and possess a high school diploma or GED. An associate or bachelor degree in social work or a clinical mental health field is preferred.
The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care.
· Minimum of one (1) year experience in behavioral field, with direct experience in discharge planning preferred.
EDUCATION / LICENSURE:
· High School diploma, GED or Higher Education required. Associate’s or Bachelor’s degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
· Crisis Prevention Certification Trainings (Handle with Care) within 30 days of employment and prior to assisting with a restraining procedure. Certification training is offered during new hire orientation.
