Case Coordinator/QIDP – Bi-Annual Retention Bonuses
If you're looking for a career that is both rewarding and deeply meaningful, DSC is currently accepting applications for the position of Case Coordinator. Developmental Services Center (DSC) provides comprehensive services across the lifespan-from birth through adulthood-including family development, case management, employment, residential, respite, and community day services for individuals with intellectual and developmental disabilities in Champaign and Ford Counties. As part of our team, you'll join a positive, caring, and mission-driven group of professionals dedicated to empowering people with disabilities to live full, self-directed lives. If you have a degree in a human services field such as Sociology, Psychology, Special Education, Rehabilitation Counseling, Recreation, or a therapy-related field, or even a designation as a COTA, COME WORK WITH US as a Case Coordinator!
RESPONSIBILITIES:
As a Case Coordinator at DSC, you will be responsible for the coordination of a wide range of supports and services for people to whom we provide services. You will need skills in service linkage, advocacy, documentation, securing and maintain entitlements and coordination of Individual Implementation Strategy. Each day, you will be engaged in rewarding work where you will play an integral role in the success of the company and our mission to support people to live a rich and meaningful life.
REQUIREMENTS:
At least one year of experience working directly with individuals with a developmental disability
AND IS ONE OF THE FOLLOWING:
RN, COTA, OTA, PTA, SLP, Audiologist, Registered Dietician, Human Services Professional with a bachelor’s degree in a human services field, including, but not limited to sociology, special education, rehabilitation counseling, psychology, recreation, art/dance/music or physical therapies. Knowledge of community resources and a demonstrated capacity to work in collaboration with other human service professionals. Valid driver’s license and reliable transportation
HOURS:
40 hour/week, 8:00 AM – 5:00 PM, Monday through Friday; One day/week remote work
