The Adult Health Home Care Coordinator develops and maintains comprehensive person-centered care plans to address the medical, behavioral, and social needs of enrollees. They lead multidisciplinary teams and provide ongoing coordination of services across community and home-based settings.
Candidates must hold a Bachelor's degree in a human services field or possess a CASAC certification with two years of relevant experience. A valid driver's license and personal vehicle with specific insurance coverage are required for travel to community locations.

The Adult Health Home Care Coordinator works with and oversees the process with individuals to develop and maintain a comprehensive plan for accessing and using services and supports necessary for a person’s health.
Pay Rate: $22.12 per hour
In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive!
