The Case Manager collaborates with interdisciplinary staff to resolve barriers to care and facilitates weekly care team conferences to review patient progress. They are responsible for maintaining accurate documentation, including Individual Plans of Care, and conducting family conferences to ensure effective communication.
Applicants must hold a bachelor’s degree from an accredited nursing or social work program. Required certifications include Basic Life Support, with ACLS and PALS certifications expected within six months of hire.

What we’re looking for
Applicants should possess a bachelor’s degree from an accredited nursing school or Social work program. Additional requirements include:
Basic Life Support certification is required within 30 days of hire.
ACLS and PALS are required within six months of hire.
One year of experience preferred
ASLS certification preferred
Salary Range: $93,600--$104,000 Per year
