The Forensic Case Manager provides comprehensive case management, treatment planning, and support services to clients with mental health challenges within a multidisciplinary team. They are responsible for implementing community service plans, managing crises, and assisting clients with daily living activities to ensure community tenure and independence.
Candidates should possess a bachelor's degree in behavioral science or at least 5 years of relevant field experience. A minimum of 3 years of experience working with individuals with mental health conditions and a valid driver's license with an acceptable driving record are required.
Payrate: $23.08-$24.04/HR

Elwyn seeks a Forensic Case Manager to be a member of a multidisciplinary team. This role will utilize your skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to (ACCS) clients under the clinical supervision of the team leader, assistant team leader, and other mental health professionals. A team member equally shares the responsibility to perform various activities and interventions that support (ACCS) clients in the community.
