Shared On-Call Rotation

One-on-One Patient Care
Focus on one patient at a time, providing personalized, compassionate care in the comfort of their home.
Independence with Clinical Support
Enjoy the autonomy of working in the field while collaborating closely with experienced RN Case Managers and a supportive clinical leadership team.

As a Home Health LVN, you'll provide skilled nursing care under the direction of an RN Case Manager while helping patients safely recover and remain independent in their homes.
Your responsibilities will include:
Deliver skilled nursing visits in accordance with the physician's orders and individualized plan of care.
Monitor patient conditions and promptly communicate changes to the RN Case Manager and interdisciplinary team.
Provide wound care, medication management, treatments, and patient education.
Reinforce disease management and self-care techniques with patients and caregivers.
Collaborate closely with RN Case Managers to ensure high-quality, coordinated care.
Document patient assessments, interventions, medications, and progress accurately and timely.
Promote patient safety while helping prevent avoidable hospitalizations and emergency department visits.
Build trusting relationships with patients, families, physicians, and care team members.
Maintain compliance with agency policies, HIPAA regulations, and clinical standards.
