As a Behavioral Health Case Manager at Noble, you will:
Deliver Compassionate Care: Provide patient-centered case management and resource navigation to support and improve overall well-being.
Be Part of an Integrated Care Model: Deliver evidence-based, quality care with the support of a collaborative team, including Medical, Pharmacy, and Dental providers.
Serve as a Case Management Expert: Work closely with an interdisciplinary team while offering your expertise to address diverse needs.
Expand Access to Care: Help break barriers to care by focusing on the needs of underserved communities.
Innovate and Collaborate: Thrive in an environment that values innovation, continuously addressing the intersection of health and behavioral health needs.
Shape the Future: Mentor and collaborate with students training at Noble, contributing to the growth of the next generation of care providers.
What You'll Do:
Provide case management and follow-up care to support patients in meeting health and treatment goals.
Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, SUD, and psychosocial needs.
Serve as a liaison across the integrated care team to improve communication and coordination of services.
Assess Social Determinants of Health (SDoH) and connect patients to appropriate internal and community resources.
Advocate for patients to help navigate systems and secure needed services.
Support patient re-engagement and care coordination activities.
Conduct secondary suicide risk assessments, develop safety plans, and assist with crisis de-escalation using trauma-informed approaches.
Facilitate integrated care clinical consult team meetings.
What We're Looking For:
Master’s degree in Social Work, or a related behavioral health field from an accredited program, preferred; Bachelor's degree in social work, with experience in case management or social services required.
Licensure Requirements: Current Wisconsin licensure as a Certified Social Worker (CSW) or Advanced Practice Social Worker (APSW), or eligibility to obtain licensure within six (6) months of hire. License must be active and in good standing with the State of Wisconsin.

The National Health Service Corps (NHSC) Loan Repayment Program (LRP) offers primary medical, dental, and mental and behavioral health care clinicians the opportunity to have their student loans repaid while earning a competitive salary in exchange for serving in urban, rural, or tribal communities with limited access to care.
