As a Field Supervisor, you’ll be the heartbeat of exceptional client care—guiding, supporting, and uplifting both clients and caregivers every day. In this role, you’ll connect with clients directly by assessing their needs and welcoming them onto our services, ensuring they feel confident and cared for from the very start.
You’ll also play a key leadership role by onboarding and orienting caregivers, setting them up for success, and providing ongoing support in the field. By overseeing the quality of care delivered, you’ll help create meaningful experiences for clients while empowering caregivers to do their best work.
Above all, you’ll serve as a trusted advocate—championing the needs of clients and attendants alike, and making a real difference in people’s lives every single day.
Job Qualifications
Education: A Field Supervisor must be a licensed nurse or have completed two years of full-time study at an accredited college or university. An individual with a high school diploma or GED may substitute one year of full-time employment in a supervisory capacity in a health care facility, agency, or community-based agency for each required year of college
Experience: A minimum of one-year successful experience in supervisory role in home health care, hospice, or related health care service administration.
Skills: Excellent organizational skills and ability to effectively handle multiple tasks required.
Professional customer service skills and time management skills required.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental/Working Conditions: Performs duties during Agency operating hours. May require work in a routine office environment; noise level may be moderately high; must have the ability to work a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc. The ability to travel locally on occasion, some exposure to unpleasant weather.
Essential Functions
Contribute to team efforts with positive attitude and in a professional manner.
Utilizing reports to monitor productivity and compliance of team members
Responsible for accurate and timely preparation and processing of HHS and department required documents; coordinates care of clients and utilizes task planners appropriately in Outlook. Ensures timeliness and completeness of all documentation.
Responsible for education to providers in client’s homes to perform HHS assigned tasks and timely preparation of documentation (timesheets.)
Establishes and maintains rapport with HHS case managers; assist with audits as required. Participates in and attends program IDT meetings as requested by HHS or Administrator. Participates in community relations activities as scheduled.
Adheres to HHS and MCO program requirements.
Available for on call rotations as required for all waiver programs.
Provides personal client assistance services in event provider staff are not available.
Performs supervisory attendant orientation visits to client homes for clients.
Maintains compliance with all At Home Healthcare policies, procedures, and regulations, including all federal and state laws.
Makes copies of various documents as required.
Maintains a clean and safe work environment.
Completes accurate and timely documentation.
Accurately prepares and proofreads letters, memos, spreadsheets and other correspondence.
Perform all other duties as requested by Management
Physical, Mental, Miscellaneous Demands
Prolonged sitting required, with the ability to operate office equipment which may include heavy lifting, bending, and standing.
Requires the ability to handle stressful situations in a calm and courteous manner at all times. If required to make patient visits, prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.
Requires working under some stressful conditions to meet deadlines and Agency needs.
Must have and maintain valid Driver’s License, and proof of current valid auto liability insurance.
Requires ability to use up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or up to 10 pounds continuously to move objects.
Work deals mostly with areas such as preparing and reading data and figures, records, reports, visual inspection involving small details. Although important, depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer.
Requires ability to grasp, pull, push, carry or otherwise manipulate objects; ability to perform tasks requiring action of muscles or groups of muscles and foot and/or hand-eye coordination.
Requires ability to communicate clearly and make self-understood effectively in face-to-face interactions; articulate with accuracy to communicate using phone.
Requires ability to hear and receive verbal instructions correctly, answer phones, communicate professionally with people in situations with some background noise.
Requires ability to concentrate on fine detail with frequent interruption; ability to focus attention on tasks for 10-20 minutes at a time on a continuous basis, 20-60 minutes on occasion.
Requires ability to understand and relate to specific ideas one at a time and to the concepts behind specific ideas; ability to remember verbal and written tasks/assignments from a few hours to several day periods.
Position requires sitting approximately 60% of time standing/stooping/bending/ climbing approximately 20% of time; and walking approximately 20% of time.
Excellent computer software skills necessary to produce accurate documents and materials required.
Working knowledge of MS Office, Excel or similar software required. Proficient knowledge preferred.
Requires ability to work effectively under minimal supervision; exercise excellent, sound professional judgment and maintain confidentiality.
