You'll work alongside clinical leaders, operational teams, public health partners, and community organizations to implement programs that make a measurable difference in the lives of the people we serve.
What You'll Do
As the Population Health Project Manager, you'll provide strategic and operational leadership for initiatives that improve preventive care, chronic disease management, community wellness, and access to healthcare resources.
In this role, you will:
Lead the planning, implementation, and evaluation of population health initiatives that improve community health outcomes and advance organizational priorities.
Manage multiple projects focused on prevention, chronic disease management, health equity, community benefit, and value-based care.
Develop project plans, timelines, workflows, and reporting processes to ensure successful execution and measurable results.
Monitor project milestones, performance metrics, utilization trends, and outcome measures to drive continuous improvement and informed decision-making.
Build and strengthen partnerships with healthcare providers, public health agencies, community organizations, schools, faith-based organizations, and civic leaders.
Coordinate community outreach programs, health education events, screenings, and initiatives aligned with Community Health Needs Assessment (CHNA) priorities.
Represent University Health on internal committees and external community coalitions supporting population health and community wellness.
Facilitate collaboration across interdisciplinary teams while identifying opportunities to improve operational efficiency and project outcomes.
Support grant initiatives, strategic planning efforts, executive presentations, and organizational reporting.
Develop dashboards, reports, presentations, and communication materials that demonstrate program performance and organizational impact.
Promote quality improvement, patient-centered care, health equity, and regulatory compliance throughout all initiatives.
What You'll Bring
Required Qualifications
Bachelor's degree in Healthcare Administration, Public Health, Business Administration, Community Health, Nursing, Social Work, or a related field; or an equivalent combination of education and relevant experience
Two (2) to four (4) years of experience in healthcare project management, population health, community health, program operations, or a related healthcare environment
Experience leading cross-functional projects and collaborating with diverse stakeholders
Strong organizational, analytical, and problem-solving skills with the ability to manage multiple priorities
Excellent written, verbal, presentation, and interpersonal communication skills
Proficiency with Microsoft Office Suite, project management software, and data reporting tools
Ability to exercise sound judgment, maintain confidentiality, and work independently in a fast-paced environment
Preferred Qualifications
Master's degree in Public Health (MPH), Healthcare Administration (MHA), Business Administration (MBA), or a related field
Project Management Professional (PMP) certification
Experience supporting population health, value-based care, community benefit, care coordination, or health equity initiatives
Knowledge of Community Health Needs Assessments (CHNAs), community engagement strategies, and healthcare quality metrics
Bilingual (Spanish/English)
