The Clinical Coordinator RN leads dialysis care within skilled nursing facilities, overseeing patient care delivery and supporting a team of clinicians and PCTs. Key duties include coordinating care for ESRD patients, ensuring regulatory compliance, and managing staffing and scheduling.
Candidates must hold a valid RN license, CPR certification, and have at least 18 months of RN experience with 6 months specifically in dialysis. CNN or CDN certifications and prior supervisory experience are preferred.

The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system.
DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility. In this role, you’ll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality.
The primary home location for the Clinical Coordinator is DaVita SNF Dialysis at Plaza 1475 Granite Reef Rd Scottsdale, AZ 85257.
